Session RATES:

  • One hour: $110*

  • 90 Minute: $150*

  • 2 Hour: $185*

TREATMENT FOR CHILDREN: $85* for 45-60 minutes.

BIODYNAMIC STUDENT discount: $10 Discount

*Time of service payments and co-pays are due at the time of service. These rates are charged to anyone paying at time of service and not requesting that the additional service of insurance billing is provided.

Cancelation policy:

  • Full Payment is due at the time of treatment. A cancellation must be made 24 hours in advance to avoid charges. The full fee payment is required for appointments cancelled with less than 24 hours notice. If you do not arrive for your appointment within 20 minutes of the scheduled time, it will be considered canceled.

Workshop policy:

  • All deposits for workshops are non-refundable.  The remaining balance for workshops is due 30 days before the scheduled workshop date.  If the workshop balance is not paid in full 30 days prior to the event date, your space may be given to a student on the wait-list and the deposit forfeited.  Please communicate with us via e-mail if there are any issues with timely payment; we want to work with you to ensure you can attend the event.  If submitting a withdrawal of your workshop registration with less than 30 days prior to the start of the event, a refund less the deposit amount will be issued only if a replacement can be found from the waiting list.  If no replacement is available, the entire tuition is non-refundable.

Insurance Billing

Motor Vehicle Accidents:
(Auto Insurance PIP claims)

  • In the case that you are using health or auto insurance to pay for a portion of your care in this office, an arrangement may be made to omit payment at time of service and await reimbursement. We are often unable to predict these costs exactly, and may not know for 12 weeks up to 6 months after the date of service, once your company has processed the claim. By receive these services you accept financial responsibility for any outstanding charges that are not covered by your insurance company.

  • The prices stated in the Services menu reflect discounts for payment at time of service for clients and are for non-medical or physician referred massages and acupuncture. Medical massage and Acupuncture treatment under MVA insurance is billed in accordance with UCR (usual, customary, and reasonable) fees this service is available in 15 minutes increments with rates per unit and is based on actual time spent covering the treatment on credit, requiring hours of billing time, tracking, and collecting payment for services and requires well documented records and chart notes for billing purposes which is normally not required.

  • Please come prepared for your initial treatment session with the following:

    • A prescription for massage therapy (acupuncture does not require prescription) including diagnosis codes (ideally written out and also in ICD-10 form), frequency of visits ordered, expiration date and referring provider information from your doctor, chiropractor or other insurance-approved primary provider for your case.

    • Your insurance information and information on the insurance we are billing if not your insurance company.

    • Claim number

    • Adjuster's name and contact information

    • Your completed electronic intake forms or bring information needed to fill them out at the office.

ORegon Health Plan Insurance coverage

  • Please first call the phone number on the back of your insurance card to confirm that you have acupuncture benefits.

  • Inquire about any required per-authorizations or referrals needed from your Primary Care to be submitted for treatment.

  • If referral or per-authorization is required, please have your PC submit these prior to scheduling your first appointment

Out-of-Network Insurance

  • If you would like to try to bill your insurance for "out-of-network" acupuncture or massage treatments, a "superbill" can be provided that you can send in to your insurance company for reimbursement.
    PLEASE NOTE: Initial payment is required at the time of the service.

  • The phone number on the back of your insurance card can be called to inquire if you have any "out-of-network" acupuncture &/or massage benefits.

Intake Forms

  • Initial session:
    New clients are required to fill out an intake form and read and sign other required forms for their first visit.
    Forms are sent electronically. If unable to access the electronic forms, please print forms, fill them out and bring them with you to your first session -OR- scan them and send to Stacy at: lightessencehealingarts@gmail.com.
    Hard copies will be also be available as needed.

  • Treatment for Minors: Minor Release Form to be filled out by the guardian of the minor who is to receive treatment.

  • NOTE: If the initial information given on the intake form changes during the course of your treatment please inform your practitioner.
